Role title: End of Life Care Coordinator

Role type: Permanent, full time

Role grade: Social Worker (Level 4) or Clinical Nurse Consultant (Grade 2)

Shift details: Monday to Friday role with full time hours

Department and location: Palliative Care, St Vincent’s Public Hospital, Sydney

Salary: $131,585.00 to $153,392.73 + Super + Salary Packaging

About the role

  • Lead the coordination of End of Life Care across SVHNS, promoting a compassionate, supportive and integrated approach to care for patients, families and staff

  • Provide expert information, education and training across SVHNS, including Advance Care Planning, Advance Care Directives and Enduring Guardianship, while developing resources to support best practice

  • Promote a positive and supportive culture for staff involved in, or choosing not to participate in, Voluntary Assisted Dying, including through peer support, clinical supervision, pre-briefing, debriefing and reflection

  • Lead End of Life Care quality and safety activities, including monitoring compliance and accreditation standards, analysing and reporting clinical data, and supporting continuous quality improvement in line with recognised health standards

  • Chair the End of Life Community of Practice and contribute to relevant committees, working groups and networking opportunities, representing SVHNS across local, area and state forums

  • Support staff wellbeing and clinical supervision initiatives within Sacred Heart Health Service, as well as End of Life Care-related debriefing and reflection sessions across SVHNS in collaboration with the Bereavement and Supportive Services Manager

About you

  • Currently registered as a Registered Nurse Division 1 with the Australian Health Professionals Regulation Agency (APHRA) OR degree in social work from an accredited course providing eligibility for the membership of the Australian Association of Social Workers (AASW)

  • Nurses - At least 5 years full-time equivalent post registration experience, with at least 3 years full-time equivalent experience in the specialty field. In addition, the employee must have approved postgraduate nursing qualifications relevant to the field in which he/she/they are appointed or such other qualifications or experience deemed appropriate by St Vincent’s Hospital, Sydney

  • Social Workers - Experience in oncology, palliative care, end-of-life care, advance care planning or in caring for patients with complex and/or life limiting conditions

  • Experience in implementing and monitoring compliance with regards to policy and procedure

  • Demonstrated ability to work autonomously with a high degree of motivation and capability whilst collaborating effectively with stakeholders, and fostering teamwork

  • Demonstrated understanding of clinical governance health care systems including quality improvement, monitoring and analysing data.
