Sentara RMH Medical Center is hiring an RN, Integrated Care Manager, to support Pulmonary Specialists in Harrisonburg, VA.
Job Summary
- Monday through Friday, 40 hours per week
- Standard work hours are 8:00 AM to 4:30 PM
- Onsite in the Medical Office Building
The RN, Integrated Care Manager, is responsible and accountable for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients across the continuum. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation of patient/caregiver needs. Functions in one of the following practice settings: Acute Care, Service Lines, Ambulatory/Community-based, Home Health, and Long-Term Care.
Experience Required:
- 3 years of RN experience Required
Experience Preferred:
- 3 years of case management/care coordination experience preferred
- Pulmonary care, thoracic surgery, oncology, complex outpatient care, or chronic disease management preferred
- Patient navigation, care coordination, care transitions, discharge planning, and longitudinal patient follow-up preferred
- Clinical triage experience preferred
- Provided patient education and collaborating with multidisciplinary teams preferred
- Strong critical thinking, barrier assessment, and care planning skills preferred
Education:
- RN Diploma Required
- ASN, BSN or MSN preferred
Certification:
- VA Registered Nurse License or compact Required
- BLS required within 90 days of hire and maintained thereafter (if in clinical setting)
- For Integrated Care Management departments, specialty certification required within one year of eligibility (ACM, CCM, CCCTM, CMAC or CGMT-BC).
- For other service lines, certification based on specialty area required within one year of eligibility.
- For those in Behavioral Health - Certification in de-escalation training within 15 days of hire and annually.
