Description
We are seeking a compassionate and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient primary care team. This is an excellent opportunity for a provider who is passionate about delivering high-quality, patient-centered care and improving access to helathcare within the community.
The position is 100% outpatient. You will work as part of a collaborative, multidisciplinary team focused on providing comprehensive primary and preventive care to a diverse patient population.
Key Responsibilities:
- Provide comprehensive outpatient primary care to patients across the lifespan, as appropriate to training and experience
- Evaluate, diagnose, and manage acute and chronic medical conditions
- Perform routine physical examinations and preventive health screenings
- Develop and implement individualized treatment plans
- Order and interpret appropriate laboratory and diagnostic studies
- Prescribe medications and treatments within scope of practice
- Educate patients regarding preventive care, chronic disease management, medications, and healthy lifestyle choices
- Coordinate care with Physicians, behavioral health professionals, MA staff, specialists and community resources
- Maintain accurate and timely documentation in the electronic health record
- Support quality improvement initiatives and FQHC clinical performance measures
- Provie culturally responsive, compassionate care to underserved and medically vulnerable populations
Qualifications:
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current or eligible NP or PA licensure in Georgia
- Current national board certification
- DEA registration or eligibility to obtain
- Current BLS certification
- Strong clinical judgement, communication, and interpersonal skills
- Ability to work effectively within a team-based care environment
- Commitment to serving diverse and underserved patient populations
- Primary care, Family medicine, internal medicine, or community health experience preferred
- New graduates may be considered depending on training and clinical experience
