Your Mission

The Case Manager is a nurse-led professional who supports members with complex medical, psychosocial, or care-coordination needs. With the member at the centre of everything we do, the Case Manager helps individuals navigate their healthcare journey with confidence, ensuring they receive the right care, at the right time, and in the most appropriate setting.

Based in Europe, the Case Manager guides the member journey from assessment through follow-up, developing personalised care plans and coordinating providers and services. Through specialised clinical programmes, the role supports members with complex care needs, promotes continuity of care and strong clinical outcomes, and helps ensure healthcare is appropriate, sustainable, and delivers meaningful value.

Working within an international and multicultural environment, the Case Manager builds trusted relationships with members, providers, physicians, clients, and internal partners, serving as a key coordinator and advocate throughout the care journey.

Key Responsibilities

• Member Advocacy & Care Coordination: Build trusted relationships with members and serve as their primary point of contact throughout the care journey. Coordinate healthcare providers, hospitals, specialists, caregivers, and support services to ensure a seamless and member-centred experience.
• Assessment & Care Planning: Assess medical, psychosocial, and functional needs, identifying barriers to care, gaps in treatment, and potential risks. Develop, implement, review, and adapt personalised care plans that support members' goals and clinical needs.
• Clinical Management & Monitoring: Monitor members' progress, support safe transitions of care, and proactively identify emerging needs or risks. Collaborate with physicians, providers, and internal clinical experts to ensure appropriate and effective care management.
• Education & Empowerment: Help members understand their medical conditions, treatment options, available resources, and self-management strategies, enabling informed decision-making and greater engagement in their health.
• Collaboration & Programme Delivery: Work closely with internal teams, external partners, and global healthcare professionals to deliver specialised clinical programmes, including Case Management, Chronic Condition Management, Lifestyle Coaching, Pre-Assignment Assessments, and other Global Clinical Operations programmes.
• Quality, Documentation & Compliance: Maintain accurate and timely case records, promote continuity and quality of care, and ensure compliance with professional standards, regulatory requirements, and organisational policies.
• Value-Based Care: Promote clinically appropriate, sustainable, and cost-effective healthcare solutions that balance member needs, outcomes, patient advocacy, and responsible use of healthcare resources.

Skills and Qualifications

• Bachelor's Degree in Nursing.
• Minimum 3-5 years of experience in Case Management, Clinical Management, or a related healthcare setting, preferably within an international environment.
• Fluent in English. Proficiency in additional languages, particularly French, Portuguese, or German, is considered a strong asset.
• Strong clinical assessment and care coordination skills.
• Demonstrated organisational, leadership, and prioritisation capabilities.
• Excellent interpersonal, communication, and relationship-building skills.
• Strong analytical and problem-solving abilities.
• Ability to advocate for members while maintaining compliance with clinical, contractual, legal, and organisational requirements.
• Ability to work independently and effectively in a dynamic, multicultural environment.
• Proficiency with Microsoft Office applications and healthcare management systems.
• Ability to collaborate effectively with healthcare providers, clients, internal stakeholders, and global teams.
