Overview
Act as liaison and advocate between UC Health patients and community healthcare providers, promoting healthcare access through patient education, mentoring, coaching and outreach. Identify patients needing primary care, community resources, and care coordination to meet social, physical and developmental needs. Requires Ohio Certified Community Health Worker Certification (CCHW), 0-11 months healthcare experience, and strong communication skills. Full-time first shift position.
