Nurse Practitioner – Palliative Care

Location: Somerset, New Jersey

A growing palliative care organization is seeking an experienced Nurse Practitioner to provide high-quality patient care while helping lead and mentor a team of advanced practice providers across New Jersey.

This hybrid role combines direct patient care with clinical leadership and offers the opportunity to make a meaningful impact on patients living with serious illnesses and complex chronic conditions.

Responsibilities

• Provide palliative care assessments and follow-up visits
• Manage patients with serious illness and complex chronic conditions
• Facilitate goals-of-care and advance care planning discussions
• Collaborate with physicians, specialists, hospitals, and community providers
• Mentor and support Nurse Practitioners and clinical staff
• Participate in quality improvement and operational initiatives
• Assist with onboarding, training, and team development

Qualifications

• Active New Jersey Nurse Practitioner license
• National board certification and DEA registration
• Minimum 5 years of clinical experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or chronic disease management
• Strong communication and leadership skills
• Experience working within interdisciplinary care teams

Preferred

• Leadership or supervisory experience
• Hospice & Palliative Care certification (ACHPN)
• Experience with home-based care, telehealth, or value-based care models
• Bilingual skills are a plus
