General Job Description:
Support our individuals with all of their needs. Ensure their health & safety needs. Assist them with their vocational tasks, be a mentor and example to all.
Requirements
MAJOR DUTIES AND RESPONSIBILITIES:
· Provide training, supervision, instruction and support to individuals in all agency settings, on site or in the community.
· Ensure that you are aware where your assigned individuals are during the day, let another staff know that you are leaving the floor and ensure that they have taken responsibility for your individuals until you get back.
· Attend worker, program, and agency required trainings and meetings and give input on individual plan development.
· Assist to develop and track all worker goals and other daily documentation
· Be aware of all Mealtime Procedures and always supervise individuals during meals
· Provide transportation as needed for outings
· Provide on-going necessary support as indicated by their IP for each worker. Assist individuals to complete tasks as independently as possible assisting if needed for the task to be completed properly
· Assist with securing community-based activities/ worksites for your individuals.
· Assist staff & individuals during fire drills or other emergency situations
· Pass medications if applicable
· Assist in providing a positive program environment to staff & participants
· Complete and maintain all daily individual documentation and worker weekly notes.
· At the end of each month remove all data collection sheets and weekly notes from each person’s book that you are assigned that day.
· Ensure completion of all entries and turn into the Assistant Coordinator each month
· Ensure all extra documents are filled out by end of day if applicable, i.e.: 255, Seizure protocol, behavior data, ABC’s, minor incidents, etc.
· At the beginning of each month ensure that the new month’s data collection pages and weekly notes are put into the books that you are assigned that day
· Sign off on COMPLETED Paylocity punches and submit with any mileage forms to the Assistant Coordinator by Friday PM
· Submit your Auto Insurance Declaration page to Human Resources upon renewal or every 6months
· Document appropriately in all Travel Books when using Company Vehicles
· It is your responsibility to let your immediate supervisor know if any of your trainings are getting ready to expire. Please give them 60 days’ notice of expiration
· Have knowledge & follow all the participants water safety, fall risks, behavior, toileting & LON requirements
· Provide assistance to peers as needed
· Assist in providing a positive program environment for staff and participants
· Assist staff & individuals during fire drills and other emergency situations
· Be a team player
· Learn & follow all agency P&P
· Learn & follow all emergency & safety procedures for the agency
· Be involved in OWC fundraising efforts
· Perform other duties as they arise
· Maintain mutual respect & professionalism for all co-workers & individuals daily
QUALIFICATIONS FOR THE JOB:
· GED/High School diploma
· 3 years’ experience in a DSO setting
· Must be reliable, dependable, and flexible with your hours
· Must possess proven experience and success
· CPR/First Aid, Med Cert, PMT a plus
· Must possess valid driver’s license and have 5 years’ experience clean driving
· Must be able to work as part of a team as well as be independent
