Capstone Hospice is in search of a Social Worker to join our dedicated hospice team covering the territory of North Gwinnett areas of Suwanee / Lawrenceville. This position is full-time. 


The Social Worker provides a wide range of psychosocial services to terminally ill/chronically ill patients and their families including regularly assessing the patient and family's psychosocial needs, pre-bereavement/anticipatory grief needs and coordination of care. The position works in the field providing these services in residential care, assisted living facilities, personal care homes and private home setting. This position will report to the Social Services Manager.

Candidate Application Process:

After an in-person interview, candidates for consideration will be subject to a background check, pre-employment physical and drug screening.



Requirements

Candidates must meet the following requirements to be considered:

· Licensed degree in social work; LMSW or LCSW.

· Minimum 2 years of experience as a social worker.

· Hospice experience preferred.

· Acceptance of and adaptability to diverse family environments, different socio-economic, cultural, racial and religious modes.

· Must have current valid driver's license, auto liability insurance and reliable transportation.

· Must provide evidence of annual TB test, COVID vaccination at time of hire.


Candidates should possess the following abilities to be successful in this role:

· Ability to work independently.

· Ability to make accurate, and at times quick judgements.

· Ability to respond to crisis outside of an office/hospital setting.

· Ability to work within an interdisciplinary setting as part of a team.

· Ability to be flexible, innovative and possess good interpersonal skills.

· Ability to cope with mental and emotional stress and demonstrate emotional stability.


Essential functions of a hospice social worker:

Provide a wide range of psychosocial services to terminally ill/chronically ill patients and their families including regularly assessing the patient and family’s psychosocial needs, Pre-Bereavement/anticipatory grief needs and coordination of care. Providing education, outreach and referral as needed.


Create and implement care plans to meet needs which include, but are not limited to:

  • Understanding of Hospice/Palliative care.
  • Completing Advanced Directives, funeral/cremation planning.
  • Facility collaboration.
  • Level of care changes.
  • Environmental safety/fall risk management.
  • Family dynamics/caregiving needs.
  • Signs and symptoms of death/decline,
  • Identifying abuse/neglect potential.
  • Psychosocial well-being (anxiety/depression), improving quality of life and decreasing stressors, care planning for suicide risk/ideation.
  • Assistance with financial resources and Veteran benefits.
  • Adhere to company policies and procedures as well as regulatory compliance requirements and directives.
  • Performs initial psychosocial and pre-bereavement assessments and assists in the development and implementation of the goal-directed interdisciplinary plan of care.
  • Ongoing assessment and updates to care plans to address ongoing needs.
  • Provides short term crisis intervention and individual or family counseling when indicated.
  • Observes, assesses, and brings to IDG conferences information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family.
  • Assumes the active role of advocate for the patient/family unit.
  • Develops and maintains contact with appropriate community agencies and services to promote interagency cooperation and to facilitate related referrals.
  • Documents comprehensive psychosocial assessment clearly and concisely and in a timely manner.
  • Accurate documentation of visits, phone contacts and referral actions per agency policy.
  • Actively participates in staff meetings, department meetings, team meetings, case conferences, in-services, and other meetings as assigned and appropriate.
  • Completes all orientation requirements. Participates in orienting new staff as needed.
  • Assumes responsibility for personal and professional development of skills.
  • Maintains all appropriate state licensing requirements.
  • Assure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.


Working Conditions:

Be able to tolerate exposure to elements, including, but no limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.


Physical Requirements:

Requires minimal physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.


Mental Requirements:

Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.


This is a full-time position with the expectations of a 40-hour work week.
