  • Assess residents and recognize, evaluate, and respond appropriately to changes in condition.
  • Notify physicians and responsible parties of significant changes as appropriate.
  • Administer medications and treatments according to orders, care plans, and facility policies.
  • Monitor medication passes and treatment schedules for accuracy and completion.
  • Maintain accurate, timely, and complete nursing documentation.
  • Perform vital signs, assessments, IV starts, and specimen collection as ordered and within scope of practice.
  • Assist with admissions, transfers, and discharges as needed.

  • Collaborate effectively with CNAs, LVNs, RNs, physicians, therapy staff, and other departments.
  • Coordinate resident care and help ensure continuity of care throughout the shift.
  • Communicate clearly and professionally with residents, families, staff, and leadership.
  • Support nursing staff in prioritizing resident needs and maintaining quality care standards.
  • Help maintain a positive, respectful, and team-oriented work environment.

  • Organized, adaptable, and able to prioritize effectively.
  • A positive team player who works well with different staff members.
  • Professional, approachable, and respectful.
  • Comfortable working independently while knowing when to seek assistance.
  • An effective communicator with strong clinical and interpersonal skills.
  • Committed to high standards of safety, compliance, and quality care.
