Occupational Therapist OT Home Health (PRN)

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health OT Occupational Therapist to work with us to provide exceptional and individualized care in Manatee county (Bradenton, Lakewood Ranch, Anna Maria Island, and surrounding areas).

Locations: Home health visits in Manatee county (including Bradenton, Lakewood Ranch, Anna Maria Island, and surrounding areas).

If desired, visits may also be available in Sarasota, Pinellas, or Hillsborough counties. We encourage you to apply!

Schedule: PRN; 5-15+ home health visits per week.

What You’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

• Active Florida OT license
• Minimum of 1 year experience as a licensed Occupational Therapist (OT).
• Home health care experience preferred.
• Strong assessment, critical thinking, and patient education skills
