HealthPlus Staffing is seeking an experienced Nurse Practitioner for a full-time opportunity providing comprehensive primary care to older adults in Stockton, California.
This position operates within a PACE (Program of All-Inclusive Care for the Elderly) model and focuses on providing coordinated, patient-centered care to a frail and elderly population.
The Nurse Practitioner will work closely with the Medical Director and interdisciplinary care team while providing direct patient care, developing individualized care plans, and coordinating care across multiple settings.

Clinical Opportunity
-
Perform initial and ongoing comprehensive patient assessments
-
Obtain medical histories and perform physical examinations
-
Develop and implement individualized plans of care
-
Diagnose, treat, and monitor acute and chronic medical conditions
-
Order and interpret laboratory and diagnostic testing
-
Prescribe medications within licensed prescriptive authority
-
Perform appropriate therapeutic procedures, including injections and immunizations
-
Provide preventive care and patient education
-
Refer patients to specialists when clinically appropriate
-
Coordinate care with specialists and other healthcare providers
-
Assist with care coordination for hospitalized patients
-
Participate in discharge planning with the interdisciplinary care team

Coordinate primary care for participants in nursing facilities
-
Conduct home visits when clinically necessary

Maintain accurate and timely clinical documentation
-
Participate in quality assessment and performance-improvement initiatives

Base Salary: $140,000 – $160,000 annually

Qualifications
-
Graduate of an accredited Nurse Practitioner program
-
Current California RN and Nurse Practitioner licenses
-
DEA registration required within six months of hire
-
Current CPR/BLS certification
-
First Aid certification
-
Five years of nursing experience
-
At least one year of experience working with a frail or elderly population

Supervisory experience preferred

Interdisciplinary healthcare team experience preferred

Advanced physical assessment skills
-
Strong primary care and chronic disease management skills
-
Ability to manage complex medical conditions in an older adult population
-
Knowledge of applicable state and federal patient-care regulations
-
PACE or community-based healthcare experience preferred

Strong communication and interpersonal skills
-
Ability to work effectively within a multidisciplinary healthcare team

Flexibility to provide care across different clinical settings as needed
