Cal-AIM Program Coordinator

• Ensures compliance with the Medi-Cal Transformation Justice-Involved Reentry Initiative requirements as outlined in the California Department of Health Care Services (DHCS) Policy and Operational Guide for Planning and implementing the Medi-Cal Transformation Justice-Involved Reentry Initiative. 
• Oversees compliance with pre-release activities, including but not limited to: screening for pre-release services, screening for behavioral health linkages, Medi-Cal billing and provider enrollment, pre-release care management, clinical consultation, medications and medication administration, Medication-Assisted Treatment (MAT), dedications and Durable Medical Equipment (DME) upon release, data sharing, and reporting and oversight. 
• Plans, assigns, supervises, reviews the work of all pre-release care managers, including but not limited to providing supervision and authorization of all Medi-Cal billing submissions linked to the pre-release care managers and/or all their care management activities.  
• Plans, organizes, assigns, supervises and reviews the work of support staff and/or volunteers.
• Collaborates with multidisciplinary staff within each correctional facility, including custody, healthcare, and social services staff, whose responsibilities contribute to achieving and maintaining Reentry Initiative compliance.
• Works collaboratively with other public offices and agencies as well as private/non-profit community organizations on Reentry Initiative activities, including Behavioral Health, Social Services, healthcare vendors, and Managed Care Plan and Enhanced Care Management (ECM) providers.  
• Provides guidance on each department’s goals, objectives, policies, and procedures related to the Medi-Cal Transformation Justice-Involved Reentry Initiative. 
• Collaborates with Sheriff’s Office and Probation Department leadership to evaluate the overall provision of services, including methods and procedures for effectiveness and consistency with program goals and objectives of achieving and maintaining Reentry Initiative compliance and offers recommendations for process improvements. 
• Oversees and contributes to the process for recruiting, hiring, and onboarding staff whose roles will impact Reentry Initiative compliance. 
• Ensures quality improvement processes are in place to optimize Medi-Cal revenue.
• Collaborates with healthcare vendors to meet the Reentry Initiative requirements.
• Facilitates ongoing stakeholder engagement to operationalize and optimize the success of the Reentry Initiative.
• Compliance with all provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) if assigned to the Health Services Agency.
• Compliance with all relevant laws and regulations regarding confidentiality.
• Performs related work as assigned.

• Completion of the education and experience required for active California licensure as one of the following:
  • Licensed Clinical Social Worker (LCSW)
  • Licensed Marriage and Family Therapist (LMFT)
  • Licensed Professional Clinical Counselor (LPCC)
  • Licensed Psychologist (licensed by the California Board of Psychology)
  • Board-Certified Psychiatrist (Licensed Physician and Surgeon certified by the American Board of Psychiatry and Neurology)
• Two (2) years of supervisory experience in the human service and/or public safety fields.
• Experience in care management, direct service provision, and/or providing social services in areas of healthcare or/and behavioral health is desired.
• Experience with coordinating care across multiple service providers, ensuring continuity and quality of care for individuals with complex needs is desired.
• Other combinations of education and experience may be considered.

• Possession of, or ability to obtain and maintain, a valid California Class C Driver License and a satisfactory driving record is required.  Individuals who do not meet this requirement due to a disability will be reviewed on a case-by-case basis.
• Must possess a California license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), Licensed Psychologist (licensed by the California Board of Psychology), Board-Certified Psychiatrist (Licensed Physician and Surgeon certified by the American Board of Psychiatry and Neurology)
• Must complete periodic continuing education coursework as required to maintain active licensure.
• Must follow the ethical and legal standards required to maintain licensure with the appropriate California licensing board.
• Must successfully complete an extensive and thorough law enforcement background investigation.
