  • Identify, qualify, and pursue new business opportunities by monitoring referral trends, market dynamics, funder priorities, and competitor activity.
  • Build territory/account plans that prioritize high-potential referral sources, funding partners, and community stakeholders.
  • Conduct outreach through face-to-face meetings, warm and cold calling, presentations, conference participation, and community engagement.
  • Develop and maintain a pipeline of opportunities, ensuring timely follow-up and disciplined movement through the sales cycle.
  • Bring market insights and client feedback to leadership to help inform service development, positioning, and growth priorities.

Stakeholder Engagement and Account Management (35%)

  • Build and maintain trusted relationships with clients, funding partners, health system stakeholders, referral sources, and community organizations.
  • Prepare tailored engagement plans for meetings and calls, focused on understanding client needs and positioning practical solutions.
  • Support account growth and retention by identifying client challenges, coordinating internal responses, and escalating risks or opportunities as needed.
  • Partner with operations and program leaders to address service issues, support client satisfaction, and ensure expectations are clearly understood.
  • Maintain regular touchpoints with assigned accounts and document key actions, decisions, risks, and next steps.

Revenue, Pipeline and Reporting (20%)

  • Maintain accurate CRM records, including contacts, opportunities, activities, next steps, and pipeline stage progression.
  • Support achievement of agreed growth and revenue targets for assigned accounts, geographies, or service lines.
  • Prepare pipeline updates, account summaries, and forecast inputs for business development and operational leadership.
  • Contribute to RFP responses, presentations, proposals, and partner-facing materials in collaboration with stakeholders.
  • Use data and relationship insights to recommend practical actions that improve conversion, retention, and account health.

Cross-functional Collaboration and Team Contribution (10%)

  • Work collaboratively with Integrated Care Design, Operations, Finance, Marketing, and other internal partners to support growth initiatives.
  • Share market, funder, and client insights with leadership and relevant teams to support service planning and continuous improvement.
  • Contribute to a growth-oriented culture by bringing a proactive, solutions-focused approach to relationship development and problem solving.
  • Keep internal teams informed of changes in market expectations, service opportunities, and funder/client needs.

Knowledge, Education & Experience

  • Bachelor’s degree in business administration, healthcare, healthcare administration, marketing, finance, or a related field; equivalent experience may be considered.
  • Minimum five (5) years’ experience in business development, sales, account management, or stakeholder engagement; healthcare, home care, or community care experience is preferred.
  • Demonstrated ability to build relationships, uncover client needs, and support growth through proactive prospecting and account management.
  • Experience conducting face-to-face meetings, presentations, warm and cold outreach, and relationship-building activities with clients, funders, referral sources, and/or clinicians.
  • Understanding of home and community care and how it fits within the broader health system.
  • Experience using CRM systems to document activities, manage pipeline, and track follow-up actions.
  • Proficient in Microsoft Office applications, including PowerPoint, Excel, Word, and Outlook.
  • Solid analytical, problem-solving, and decision-making skills, with the ability to translate information into business actions.
  • Business acumen and the ability to understand market needs, client priorities, and service positioning.

Technical and Interpersonal Skills

  • Strong communication, presentation, and negotiation skills, with the ability to build credibility and trust with stakeholders.
  • Ability to manage assigned accounts and opportunities independently while knowing when to escalate risks, barriers, or strategic opportunities.
  • Comfortable using CRM data, pipeline reports, and client insights to guide priorities and support decision-making.
  • Highly organized and proactive, with the ability to manage multiple priorities in a dynamic environment.
  • Solutions-oriented approach to addressing client questions, objections, service concerns, and growth opportunities.
