SUMMARY/OBJECTIVE:


The Community Outreach Liaison is responsible for developing and strengthening relationships with healthcare providers, medical practices, health systems, and community partners to increase awareness, referrals, and access to Salience Health services. This relationship-driven business development role focuses on provider outreach, education, presentations, networking, and consistent follow-up to develop new referral opportunities and strengthen existing partnerships. The Community Outreach Liaison manages an assigned geographic territory, identifies referral opportunities, develops account strategies, tracks referral and account performance, and collaborates with internal teams to support referral growth and patient access. The ideal candidate is personable, proactive, organized, driven, and comfortable working independently in a field-based environment while meeting measurable business-development goals.


RESPONSIBILITIES/DUTIES


Provider & Community Relationship Development

  • Develop and maintain strong professional relationships with physicians, advanced practice providers, therapists, practice managers, healthcare leadership, and community partners.
  • Conduct consistent in-person visits, provider meetings, educational presentations, networking activities, and follow-up communication with referral sources.
  • Establish Salience Health as a trusted clinical and community resource within the assigned geographic territory.
  • Develop meaningful, long-term relationships with providers and practice leaders rather than focusing solely on activity or visit volume.
  • Maintain consistent communication and follow-up with existing and prospective referral sources.
  • Represent Salience Health professionally at community events, provider meetings, networking events, healthcare organizations, and other relevant activities.

Business Development & Referral Growth

  • Identify new referral opportunities within primary care, psychiatry, behavioral health, specialty practices, ACOs, health systems, and other strategic healthcare partners.
  • Educate referring providers and healthcare organizations regarding Salience Health services, patient access, referral processes, clinical models, and appropriate patient populations.
  • Develop strategies to establish new referring provider accounts and convert relationships into active referral sources.
  • Create and execute account-specific strategies for high-value referring practices.
  • Strengthen and protect existing high-value referral accounts and identify opportunities to increase account activity.
  • Identify declining, growing, and high-opportunity accounts and develop appropriate outreach strategies.
  • Support growth of Salience Health referrals, including external TMS referrals and other applicable service referrals.

Territory & Account Management

  • Independently manage an assigned geographic territory and prioritize activities based on referral volume, growth potential, relationship strength, account performance, and business opportunity.
  • Track provider visits, outreach activity, referrals, account growth, opportunities, and other business-development activities within CRM and reporting systems.
  • Analyze territory and account performance to identify trends, opportunities, declining accounts, and areas requiring additional engagement.
  • Maintain organized and accurate records of referral sources, account activity, follow-up, opportunities, and relationship development.
  • Develop and maintain a pipeline of prospective referring providers, practices, healthcare organizations, and community partners.

Internal Collaboration & Operational Support

  • Collaborate closely with clinical, marketing, intake, operations, and leadership teams to support referral growth, referral conversion, and patient access.
  • Communicate provider and community feedback to appropriate internal departments.
  • Identify barriers to referral conversion or patient access and communicate opportunities for improvement to appropriate leadership.
  • Support departmental and organizational business-development initiatives and strategic priorities.
  • Meet or exceed established monthly and quarterly referral, activity, account-development, and territory performance goals.
  • Perform other duties and special projects as assigned.

REQUIRED EDUCATION/EXPERIENCE:

  • High School diploma or equivalent required.
  • 2+ years of experience in healthcare, medical sales, physician relations, business development, community outreach, or a related field.
  • Demonstrated experience building professional relationships and developing new business, referrals, partnerships, or accounts.
  • Experience working independently and managing multiple priorities, accounts, or a geographic territory.
  • Experience meeting measurable performance goals or business-development objectives preferred.
  • Experience using CRM systems, Microsoft Office, and basic reporting tools preferred.


PREFERRED QUALIFICATIONS:

  • Experience working with physician practices, health systems, ACOs, behavioral health, primary care, or specialty healthcare.
  • Existing professional relationships with healthcare providers or organizations in the North Texas market.
  • Experience in behavioral health, psychiatry, TMS, primary care, or value-based care.
  • Experience presenting to groups of healthcare professionals.
  • Experience managing a book of business, referral territory, geographic territory, or strategic accounts.
  • Bachelor's degree in business, healthcare administration, marketing, communications, or related field.

PERFORMANCE EXPECTATIONS:


Performance will be evaluated using measurable territory and business-development outcomes, including new Salience Health referrals and leads; external TMS referrals; growth of existing referring accounts; new referring provider accounts; strategic account development; provider engagement and face-to-face activity; referral trends by account; referral-to-appointment conversion; account retention and relationship strength; and monthly and quarterly territory performance.

A successful Community Outreach Representative will consistently grow referral volume within their territory, develop meaningful relationships with providers rather than simply completing visits, turn new relationships into active referral sources, strengthen and protect existing high-value accounts, identify opportunities before they become lost or declining accounts, communicate Salience Health's value clearly and consistently, demonstrate strong follow-through and accountability, use data to prioritize time toward the highest-return opportunities, and become a trusted resource for providers and practice leadership.
