Description
- Perform comprehensive nursing assessments and ongoing evaluations of hospice patients.
- Develop, implement, and update individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team.
- Provide skilled nursing care with a strong focus on comfort and symptom management.
- Recognize changes in patient condition and respond appropriately and promptly.
- Educate and support patients and families regarding medications, symptom management, disease progression, safety, and what to expect throughout the hospice journey.
- Coordinate care with LPNs, CNAs, social workers, chaplains, physicians, facilities, pharmacies, DME providers, and other members of the hospice team.
- Communicate changes in condition and patient needs to the appropriate members of the interdisciplinary team.
- Participate in interdisciplinary group (IDG) meetings and collaborate in developing and updating the plan of care.
- Complete accurate and timely clinical documentation.
- Help ensure patients and families have the medications, equipment, supplies, education, and support they need.
- Provide guidance and supervision to LPNs and hospice aides as appropriate.
- Participate in the on-call rotation as scheduled.
- Maintain compliance with hospice regulations, agency policies, and professional nursing standards.
- Current Registered Nurse license in the State of Utah.
- Graduate of an accredited nursing program.
- Nursing experience preferred; hospice, home health, palliative care, oncology, geriatrics, or other relevant experience is a plus.
- Strong clinical assessment and critical-thinking skills.
- Compassionate and comfortable caring for patients and supporting families at end of life.
- Excellent communication and interpersonal skills.
- Strong organization, time-management, and prioritization skills.
- Ability to manage a caseload while recognizing and responding to changing patient needs.
- Ability to work independently in the field while also being an active member of a collaborative team.
- Dependable, flexible, and committed to following through for patients and families.
- Current driver’s license, reliable transportation, and automobile insurance.
