Description

TITLE: Home Health RN Case Manager (RNCM) + Recertification Nurse

LOCATION: Southeastern Ohio inc. Jackson, Gallia, Vinton, Ross, Pike and adjacent Counties (1-hour travel radius required)

Home Care Network, Inc. (HCN) is a nurse-owned and locally led home health agency, which means our patients' care and wellbeing - not profits - come first. For our employees - it also means that our agency is an environment that nurses and caregivers thrive in, allowing us to handpick the best care team members to be your peers and delight our clients. Together, we can empower Ohioans with the independence they deserve - while you make a difference with an agency where you matter.

To support our growth and the increasing volume of client care in Ohio, we are creating a unique new position: RN Case Manager (RNCM) / Recertification Nurse.

We are seeking a highly skilled, experienced, accountable, and self-motivated home health Registered Nurse (RN) to join our agency and serve existing and returning clients throughout southeastern Ohio, primarily in Jackson, Vinton, Pike, Ross, and Gallia Counties. This position requires travel to several counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or travel).

This is a long-term employment opportunity with schedule flexibility and autonomy.

More about the Role

This role requires a mix of RNCM services and recertification visits - and travel - as needed to all clients across our South / Southeast Ohio region.

  • Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. *Services may also require occasional/rare weekly visit(s).
  • At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.
  • Must have experience with OASIS and NON-OASIS assessments.
  • Desirable but not required:
    • Nurse Supervision experience
    • People management
    • Peer/staff education and development
    • Success or willingness to build relationships with client payors, case mgr / entity relationships (i.e., AAA, DoDD County Board, VA, etc.) in NW Ohio and our current + prospective employees in these communities.
  • Exact travel locations and daily shifts are customizable based on your availability and willingness to travel.
  • Typical work hours and scheduled services take place during a standard 8am - 5pm weekday schedule, however many clients attend school and cannot be seen until 3-5pm, so this role requires visits occasionally outside of this schedule.
  • Round-trip / Door-to-Door mileage reimbursement provided.

  • Weekly pay every Friday
  • Paid onboarding and training
  • Additional four hours of training pay included on your first paycheck after successful completion of your first day worked

  • Customized service areas and travel expectations
  • Schedule accommodations for continuing education
  • Eligible full-time employees may qualify for health, dental, vision, and other company-sponsored coverage plans and benefits (for Individual + Dependent(s)/Family), PTO, and more.
  • All employees are eligible for unlimited referral bonuses based on HCN's Referral Bonus Program criteria, monthly and quarterly incentives and recognition, and Growth Opportunity Support such as pay and title promotions for advanced certifications and degrees, as well as schedule accommodations for school/classes.
  • All RNs are eligible for generous referral bonuses, quarterly awards and bonuses, career advancement, and other perks.
