Case Manager

Job Title: Case Manager at Woodlake Addiction Recovery Center Woodlake Addiction Recovery Center is seeking a highly motivated and compassionate individual to join our team as a Case Manager. As a premier addiction recovery center, we are dedicated to providing our clients with the highest quality of care and support in their journey to overcome addiction. Our team is committed to making a positive impact in the lives of individuals and families affected by addiction. Key Responsibilities:• Conduct comprehensive assessments of clients' needs and develop individualized treatment plans• Collaborate with clinical staff to coordinate treatment and ensure the implementation of the treatment plan• Provide case management services, including referrals to external resources and follow-up appointments• Monitor client progress and provide guidance and support throughout their recovery journey• Communicate and collaborate with clients' families and loved ones to ensure a holistic approach to treatment• Maintain accurate and confidential records of clients' progress and treatment plans• Participate in team meetings and provide updates on clients' progress and any challenges or concerns• Keep up-to-date with industry trends and best practices to continuously improve the quality of care provided Qualifications:• Bachelor's or Master's degree in social work, psychology, counseling, or related field• Minimum of 2 years of experience in case management or related field• Knowledge of addictions, mental health, and trauma-informed care• Strong communication and interpersonal skills• Ability to work collaboratively as part of a team• Experience with electronic medical records Benefits:At Woodlake Addiction Recovery Center, we believe in investing in our employees just as much as our clients.
