POSITION OVERVIEW

We are seeking a compassionate and dedicated Case Manager to join The Good Seed team serving Los Angeles County Service Planning Area (SPA) 8. This part-time weekend position provides critical case management coverage at our Drop-In Center, ensuring continuity of care for individuals experiencing mental health challenges, homelessness, and/or co-occurring disorders.

The Case Manager plays a key role in maintaining stability and support for clients during weekend hours, serving as a bridge between weekday service teams and ensuring seamless coordination of care in a low-barrier, trauma-informed environment.

KEY RESPONSIBILITIES

• Provide on-site case management services to walk-in clients during weekend hours

• Conduct intake assessments and develop individualized service plans

• Connect clients to mental health services, substance use treatment, housing resources, benefits, and healthcare

• Maintain accurate and timely documentation and case records

• Collaborate with weekday multidisciplinary teams including clinicians, outreach staff, and peer specialists

• Support clients in navigating LA County mental health and social service systems

• Monitor client progress and communicate updates to weekday staff

• Respond to crisis situations and coordinate appropriate interventions

• Ensure accurate handoff of client information between weekend and weekday teams

• Participate in trainings, supervision, and team meetings as required

• Maintain professional boundaries while fostering supportive client relationships

• Perform other duties as assigned

QUALIFICATIONS

Required:

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field (or equivalent experience)

• 1–2 years of experience in case management, mental health services, or social services

• Knowledge of LA County SPA 8 resources and service systems

• Availability to work weekends consistently (Saturday–Sunday, 9:00 AM–3:00 PM)

• Strong communication and documentation skills

• Ability to work with individuals experiencing mental illness, homelessness, and/or substance use disorders

• Proficiency in case management systems and documentation

Preferred:

• Experience in Drop-In Center or low-barrier service settings

• Ability to work independently with minimal supervision

• CPR/First Aid certification

This position operates in a Drop-In Center setting serving a high-need population in Long Beach, CA. The role requires independent work during weekend hours in a fast-paced, dynamic environment while maintaining coordination with weekday teams.

Candidates must be able to remain calm and effective in crisis situations while demonstrating professionalism, respect, and a trauma-informed, client-centered approach to care.
