PHARMACIST, PHARMACY

ORGANIZATION & MANAGEMENT 

1. To understand and carry out pharmaceutical care in line with Vision and mission, Core Values, Goals and objective that suit the scope of the Pharmacy services and the hospital. 

2. To understand the service organization structure. 

3. To attend meetings held by the services in order to discuss issues and matters pertaining to the operation of the service. 

4. To check and sign poison purchase orders prepared by the pharmacy purchasing assistant. 

5. To perform, assists and supervises all activities in the pharmacy including dispensing, sales and supply/ dispensing of medications. 

6. To contribute to in-service training for healthcare workers and provide counseling on the use and care of prescription drugs to patients. 

7. To assess the educational needs of all the staffs and provide necessary training. Gives examination to pharmacy staffs and to test and upgrade their skills. 

8. To ensure that Dangerous Drugs and Psychotropic Drugs are handled, stored and issued according to laws and regulations and hospital policies. 

9. To issue Psychotropic Drugs to nursing units. 

10. Responsible in recording and filing of Psychotropic & Dangerous Drugs and to hold the keys of the cupboard. 

11. To ensure prescription received are screened and interpreted correctly before dispensing. 

12. To perform medication appropriateness review and validate any medication reconciliation when necessary 

13. To check and endorse inpatient orders, outpatient prescription and TTA prescription (prescription intervention). 

14. To assist the Chief Pharmacist in the evaluation of Staff as required. 

15. To handle inquiries concerning drug contents, dosages, regimen review errors, storage and disposal. 

16. To monitor drug therapies for drug interactions, allergies and contraindications. 

17. To monitor and control inventory to ensure that drugs are available when needed and wastage is minimized. 

18. To handle inquiries concerning drug contents, dosages, regimen review errors, storage and disposal. 

19. To ensure and advises on safe disposal of pharmaceutical products. 

20. To participate in quarterly and as when needed stock takes. 

21. To check the prepared roster, verifies staff’s annual leave prior to approval from Chief Pharmacist. 

22. To involve in identifying lapses/challenges in the services and contributes in solving and decision making and to conduct Root Cause Analysis in case of Medication Error. 

23. To report and document adverse drug reactions, medication errors. 

Involves in drug inventory procedures, inventory management, purchasing, receiving, drug supplies and medication storage of the services. 

24. To keep work area tidy and clean.  

25. To be precise and accurate while dispensing and in case of any error occurred to immediately report to Chief Pharmacist. 

26. To uphold best practices by using 7R rules - Right drug, right patient, right frequency, right duration, right route, right information and right documentation.

HUMAN RESOURCE AND DEVELOPMENT 

1. To take personal responsibility for own professional development and keep up to date with professional change and growth by maintaining mandatory learning hours. 

2. To participate in the training and orientation of new staff in pharmacy. 

3. To supervise all pharmacy staffs including: orientation, determining workload and delegating assignments, training, monitoring and evaluating performance. 

POLICIES AND PROCEDURES 

1. To adhere to the hospital policies, procedure, guidelines and work instructions. 

2. To adhere to all Acts, Laws, Rules and Regulation governing the pharmacy service at all time. Liaise and coordinate with Doctors and other services on issues related to Pharmacy Services. 

FACILITIES AND EQUIPMENT 

1. To ensure that all equipment in the unit are operating according to procedure.  

2. To provide assistance / guidance to new staff in using equipment. 

3. To maintain accurate records of equipment and drugs in the pharmacy. 

4. To ensure that all equipment are functioning well and service according to PPM schedule. 

5. To report to Engineering personnel if any faulty and takes necessary action accordingly. 

6. To ensure maintenance of equipment by proper cleaning, handling and utilization as directed.

7. To assist the Pharmacist in Charge in keeping all records and reports of equipment in the pharmacy. 

ENVIRONMENTAL, SAFETY AND QUALITY IMPROVEMENT ACTIVTIES 

1. To carry out duties as an employee as stipulated  in Occupational Safety and Health Act (Amendments) 2022 and Other Regulations:- 

a) To take reasonable care at work for the safety of yourself and other persons; 

b) To co-operate with your employer or any other person in the discharge of any duty; 

c) To wear or use any protective equipment or clothing provided by the employer. 

d) To comply with any instruction or measure on occupational safety and health. 

2. Participates actively in any other new service that is introduced by the services. 

3. Participates in 5S activities and comply with the specified requirements. 

4. Practices Standard People Practice (SPP) at all times.  

5. Ensure compliance of Patient safety goals as safety improvement measures recommended by National Patient Safety Goals. 

6. Policies on patient and family rights are addressed at all times. 

7.  To participate actively and encourages compliance to all policies and Procedures of ISO9001:2015, ISO 14001:2015 and OHSAS 18001:2007, HACCP Netherlands, Malaysian Hospital Accreditation Standards, Joint Commission International Accreditation Standards (JCI) and MPSG. 

ADDITIONAL DUTIES AND RESPONSIBILITIES IN THE SPECIFIC AREAS 

1. To represent Pharmacy Services in management meetings in case Chief Pharmacist not available. 

2. To assume supervisory role over pharmacy assistants. 

3. To source or to replace staffs on night shifts if there is any emergency. 

4. To be on call on rotation basis and to attend the calls when required. 

5. To source out medication in case stocks insufficient / emergency requirements during the absence of Chief Pharmacist. 

6. To be involved in various committees/ activities assigned by management. 

7. To be involved in Pharmacy Clinical Services. 

8. Performs any other duties as ordered by the Chief Pharmacist.
