Description
- Provide one-on-one physical therapy treatments in patients' homes.
- Carry out individualized treatment plans established by the supervising Physical Therapist.
- Work closely with remote supervising PTs regarding patient progress, changes in condition, and treatment recommendations.
- Help patients improve mobility, strength, balance, safety, and overall function.
- Complete, accurate, and timely clinical documentation.
- Communicate with patients, families, supervising PTs, and members of the clinical team.
- Coordinate with our administrative team regarding scheduling, patient care, and logistics.
- Manage your caseload based on your availability and employment structure.
- $35–$50 per visit, based on the tiered compensation structure.

- Home health experience is preferred but not required.
