Description
The Interim Case Manager serves as a central point of coordination for assigned student cases, helping students identify appropriate university and community resources, facilitating communication among campus partners, and supporting continuity of care. The position maintains comprehensive case records, coordinates follow-up, supports students and families in navigating university resources, and works closely with campus partners to respond to students with varying levels of need and acuity. Additional duties may include supporting other Dean of Students Office programs and initiatives.
Position Specific Accountabilities
· Serve as a primary case manager for students experiencing significant personal and behavioral health concerns, including serious mental health challenges, substance misuse, housing insecurity, food insecurity, adjustment and transition concerns, and other barriers to student well-being. Collect, organize, and evaluate relevant information, develop individualized support and intervention plans, connect students with appropriate resources, and provide ongoing follow-up.
· Maintain a comprehensive caseload of students identified as needing additional support and services. Prioritize cases based on level of need, risk, and acuity, and maintain regular follow-up appropriate to each student’s circumstances.
· Maintain accurate and timely electronic case records, including correspondence, meetings, referrals, interventions, follow-up plans, and other relevant case activity.
· Support students navigating medical leaves, personal leaves, academic disqualification, and other significant interruptions to enrollment. Serve as a resource and coordinating partner with the colleges and appropriate university offices when academic concerns intersect with broader student support needs.
· Provide transition and postvention support for students returning to campus following psychiatric hospitalization, medical leave, or other significant health-related or personal circumstances. Coordinate with appropriate campus partners to support a successful return.
· Participate as a member of the Community of Care Team and collaborate closely with campus partners, including Student Psychological Services, Student Health Services, Public Safety, Student Housing, Disability Support Services, academic colleges, Financial Aid, and other university departments to support coordinated care and continuity of services.
· Participate in routine case consultation and coordination meetings and be available for ad hoc meetings when urgent or emergency student situations require coordinated response.
· Assist students and their families or caregivers in navigating university resources and systems, including health and wellness services, disability support, financial aid, housing, academic resources, and other areas, consistent with FERPA and other applicable laws and regulations.
· Provide families and caregivers with appropriate resources, gather collateral information when appropriate, and help manage communication and family dynamics that may affect a student’s care and support plan.
· Facilitate communication, as permitted under FERPA, among identified students, campus departments, faculty, staff, parents, families, caregivers, and designated emergency contacts to support coordinated student care.
· Build and maintain positive collaborative relationships with university departments and appropriate community providers and organizations. Assist students in accessing external resources when university services are not sufficient or appropriate.
· Support campus-wide initiatives, outreach efforts, and educational campaigns designed to assist students experiencing significant personal, behavioral health, basic-needs, or other well-being challenges.
· Serve as a resource to university colleges, faculty, and staff when concerns arise regarding students who may benefit from Community of Care support and assist with appropriate referrals and coordination.
· Fulfill other responsibilities as requested.

Requisite Qualifications
· Master’s degree in Social Work, Counseling, College Student Personnel, Higher Education, or a related field.
· Minimum of two years of related experience in higher education case management, student support, crisis response, behavioral health support, or a related area.
· Preferred experience includes knowledge of laws, regulations, and professional practices pertaining to student support, including FERPA, HIPAA, Title IX, Section 504, and the Americans with Disabilities Act.
· Demonstrated experience supporting students with complex personal, behavioral health, basic-needs, and crisis-related concerns.
· Demonstrated knowledge and experience in case management, assessment, organization, planning, decision-making, problem-solving, resource coordination, and effective case follow-up.
· Experience responding to crisis situations and working collaboratively with multidisciplinary teams.
· Demonstrated sensitivity to and understanding of the needs of diverse student populations.
· Strong written and verbal communication skills, including experience documenting complex cases and preparing reports, summaries, and other case-related materials.
· Ability to establish and maintain collaborative relationships with students, families, faculty, staff, campus partners, and community providers.

Salary range
$80,600.00 - $104,800.00 Salary commensurate with education and experience.Please note that this position may not be eligible for visa sponsorship now or in the future.
