St. Francis Healthcare Center is seeking a detail-oriented, organized, and knowledgeable MDS LVN to join our team. The ideal candidate will have experience with MDS assessments, care planning, and regulatory compliance in a skilled nursing or post-acute care setting.

Key Responsibilities
- Complete and coordinate Minimum Data Set (MDS) assessments in accordance with federal and state regulations.
- Ensure MDS assessments are completed accurately and within required timeframes.
- Coordinate with the interdisciplinary team to develop and update individualized resident care plans.
- Review clinical documentation to ensure accuracy, completeness, and compliance.
- Monitor assessment schedules, including admission, quarterly, annual, and significant change assessments.
- Support accurate Resident Assessment Instrument (RAI) documentation and care planning processes.
- Collaborate with nursing, rehabilitation, dietary, social services, and other departments.
- Assist with ensuring accurate documentation and appropriate reimbursement through compliant assessment practices.
- Maintain compliance with CMS requirements, state regulations, and facility policies.
- Participate in audits, quality assurance activities, and performance improvement initiatives.
Qualifications
- Current and active California Licensed Vocational Nurse (LVN) license.
- Previous MDS experience in a skilled nursing or post-acute care facility preferred.
- Knowledge of CMS regulations, RAI guidelines, and MDS assessment requirements.
- Strong clinical documentation, organizational, and time-management skills.
- Excellent communication and collaboration abilities.
- Ability to manage multiple deadlines while maintaining accuracy and attention to detail.
