Assessment Counselor

• Salary Range: $52,000 - $60,000 per year 
• Schedule: Full time, 40 hours/week, in-person, Monday-Friday
• Required Licensure: LPC, LCSW, LCADC

Essential Duties and Responsibilities 

• Conduct screenings and level of care assessments utilizing clinical knowledge, professional judgment, and approved assessment tools while communicating the assessment process with clients. 
• Evaluate behavioral, emotional, cognitive, spiritual, social, environmental, and recovery needs of clients and family members. 
• Collect demographic, historical, psychosocial, and clinical information relevant to level of care determinations and admission recommendations. 
• Gather information and complete pre-certifications, utilization reviews, and payer authorization requests as required. 
• Facilitate admissions by coordinating with appropriate clinical, medical, nursing, and operational team members. 
• Coordinate referrals and linkage to community providers and assist clients in obtaining placement in the most appropriate treatment setting. 
• Complete assessments, admissions documentation, releases of information, treatment recommendations, and other required documentation accurately and within established timeframes. 
• Collaborate with treatment team members and departments to support continuity of care, maintenance of clinical records and admission processes. 
• Conduct brief individual therapeutic interventions to engage clients in treatment and document services appropriately. 
• Provide case management and linkage to alternative services when indicated. 
• Develop initial treatment plans and recommendations based upon assessment findings. 
• Facilitate psychoeducational, therapeutic, and process groups as assigned. 
• Participate in interdisciplinary treatment team meetings, case reviews, and clinical discussions as assigned. 
• Provide coverage for other clinical team members as operationally necessary. 
• Must exercise discretion and maintain confidentiality with regard to all company information.  
• Completion of all required trainings as designated by the company and accreditation/licensing entities.  
• Other duties as assigned. 

Education/Credentials:

• REQUIRED LICENSURE: LPC, LCSW, LCADC.
