Description
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments.
- Help patients improve strength, balance, mobility, endurance, safety, and functional independence.
- Provide therapeutic exercise, gait training, transfer training, balance activities, and fall-prevention interventions.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Recommend assistive devices and adaptive equipment when appropriate.
- Educate patients and caregivers on exercises, mobility strategies, and safety.
- Complete accurate and timely clinical documentation.
- Communicate with patients, families, physicians, facility staff, and interdisciplinary care team members.
- Coordinate with Stern's administrative team regarding scheduling, patient care, and logistics.

- Geriatric, assisted living, skilled nursing, outpatient, or home health experience preferred but not required.
