Job Summary

Case Managers play a critical role in the multi-year grant-funded program designed to transition disconnected youth and adults with disabilities into competitive, integrated employment using holistic family supports. As an integral part of the support team, the Case Manager works to move families toward self-sufficiency by linking individuals and families to essential community resources, coordinating referrals and applications, and navigating barriers to employment. Using a wrap-around approach, the Case Manager develops care plans, continuously assesses changes in status and support needs, and updates care plans as necessary to ensure ongoing support. This grant position is expected to end in early 2029.

Primary Duties

  • Conduct comprehensive assessments to identify support needs and establish individualized and family goals, when applicable, for achieving self-sufficiency. Collaborate with team members such as employment development specialists, youth transition specialists, vocational rehabilitation counselors, TANF case managers, and school counselors to ensure coordinated support.
  • Develop, implement, and regularly update person-centered plans tailored to each participant’s needs, ensuring ongoing alignment with their evolving circumstances and employment goals.
  • Link participants to vital community resources, assist with locating appropriate services, and provide guidance on navigating complex systems to access those resources.
  • Monitor support needs and address barriers to accessing services, proactively working with participants to overcome obstacles that hinder their pathway to employment and independence.

Additional Responsibilities

  • Build and maintain long-term relationships with community partners, participants, their families, and other stakeholders (e.g., medical teams, paid and natural supports, legal guardians) to ensure access to up-to-date resources and support.
  • Regularly communicate and collaborate with state vocational rehabilitation counselors.
  • Stay informed on current and emerging community resources, including programs and services related to healthcare, housing, education, employment, and other relevant supports.
  • Build and maintain partnerships with local organizations to ensure access to the most up-to-date information, enabling effective referrals and support for individuals and families.
  • Record participants’ progress including charting referrals, home visits and other notable interactions
  • Facilitate and participate in team meetings.
  • Identify and/or provide support resources related, but not limited to independent living, aging in place, medical support, mental health support, emergency response plans/support, in-home companion care, housing, social and leisure activities, transportation, and financial planning and needs.
  • Actively seek opportunities to enhance knowledge of systems supporting TANF recipients and disconnected youth, including the barriers they face. Foster collaboration through participation in community meetings, professional networks, and relevant trainings, enabling effective referrals and comprehensive care for individuals and families.
  • Regularly meet with the Project Director, Project Evaluator, and key team members, including case managers, youth transition specialists, and employment development specialists from ServiceSource and affiliated organizations, to ensure alignment and progress toward grant objectives
  • Other duties as assigned.

Qualifications: Education, Experience, and Certification(s) 

  • Bachelor’s degree in social work, psychology, or human services required, master’s degree in social work, recreation therapy, counseling, or a related human services field preferred.
  • A minimum of 2-5 years of experience working with individuals with disabilities, including transition-age youth, children, adults, and families, is required. Experience working with TANF participants and families receiving TANF support is preferred
  • A minimum of 2-5 years of experience in case management, care coordination, and the development and implementation of individualized plans is required.
  • Annual criminal background checks, child abuse clearances, and other required screenings in compliance with ServiceSource, state, and funding agency regulations.
  • Valid state driver’s license (per state law) and/or access to reliable transportation for work-related travel required. Eligible drivers must maintain a good motor vehicle record (MVR).

  • Proficiency in Microsoft Office products.
  • Strong verbal and written communication skills.   
  • Excellent organizational skills.
  • Detail-oriented and able to carry out work with the highest levels of accuracy. 
  • Committed to social inclusion model and strengths-based approach.
  • Knowledge of continuum of care and transition planning.
  • Ability to establish and maintain positive working relationships with participant systems.
  • Knowledge of public benefits (SNAP, TANF, Medicaid, Medicaid Waivers, and Medicare, etc.) preferred.

While performing the duties of this job, the employee is regularly required to walk, sit, and stand; reach with hands and arms; and talk, see, and hear. The employee may often or occasionally lift or move durable medical equipment, supplies, or groceries up to 50 pounds. The employee may often or occasionally aid participants with balance (i.e. provide an arm to keep steady) and aid with transferring to and from a wheelchair and/or vehicle. Specific vision abilities required by this job include close vision, distance vision, depth perception, and ability to adjust focus. The ability to walk up and down flights of stairs is required.

  

Work Environment

The noise level in the work environment is usually quiet or moderate; however, loud noise may be encountered. Job duties may be required to be performed during inclement weather and during any degree of outdoor temperatures.
