Position Summary

Provides prescribed nursing care services to terminally ill patients in a home setting, nursing facilities, assisted living facilities and/or hospitals. Provides education and support for family members and caregivers. Functions independently without on-site supervision.


Position Specific Responsibilities

  • • Demonstrates knowledge and competency in performing baseline nursing skills.

    • Maintains clinical competencies according to agency standards.

    • Performs accurate assessments, develops and implements a written Plan of Care with the patient, following physician’s orders and according to Agency guidelines.

    • Performs and documents nursing interventions including patient education according to the Plan of Care and adapts these actions as patient needs change.

    • Addresses patient needs and problems, nursing intervention and patient response, safety interventions and education and teaching.

    • Administers all medications and treatments according to agency policy and procedure, accepted standards of care and agency guidelines.

    • Communicates effectively with patients, families, caregivers, and physicians.

    • Documents patient status in an accurate, timely and concise manner in electronic medical record, as required by Agency policy.

    • Demonstrates organization and leadership abilities by effective use of time and personnel through supervision and teaching.

    • Effectively establishes priorities and delegates responsibilities appropriately when supervising the LVN and the CNA.

    • Participates in case conferences/team meetings.

    • Makes visits as scheduled and participates in on-call rotation as required.

    • Maintains a committed and cooperative attitude with staffing, promoting teamwork, effectiveness and efficiency.

Education and/or Experience

  • Required - Graduate of an accredited school of nursing program.
  • Required - Coursework/Training Appropriate competency assessments completed.
  • Preferred - Bachelor's Degree Nursing.
  • Required - 1 year Related experience.
  • Preferred - 1 year Experience in hospice.


Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
  • Language Skills: Advanced communications skills (verbal and written).
  • Computer Skills: Ability to operate standard office equipment and electronic medical record.
  • Other Skills and Abilities: Assessment skills related to patient and family care.
  • Other Skills and Abilities: Ability to respond to common inquiries or complaints, regulatory agencies or members of the business community.
  • Other Skills and Abilities: Time management and organizational skills.
  • Other Skills and Abilities: Cooperative attitude.
  • Other Skills and Abilities: Ability to work with a variety of healthcare professionals across a wide variety of healthcare settings.
  • Other Qualifications: No listing on OIG Excluded Providers List.
  • Other Skills and Abilities: Extensive local travel in personal vehicle to provide service delivery.
  • Other Skills and Abilities: Exhibits flexibility and adaptability in a dynamic and fluid environment.


Certifications, Licenses, and Registrations

  • Required upon hire: Texas Registered Nurse License (Vendor Managed)
  • Required within 30 days of hire: National Provider BLS - American Heart Association (Vendor Managed)


Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
