Overview
As a Rehabilitation Specialist, you'll work directly with individuals referred from CCBDD and Medicaid funding sources to help them develop essential daily living skills and job readiness abilities. Your responsibilities include implementing personalized programming, maintaining detailed case notes, coordinating with support administrators, and conducting community engagement activities. You'll manage a caseload of up to 12 individuals, document progress, and ensure all activities comply with Medicaid and CARF requirements. The role requires driving agency vehicles for community outings and maintaining professional communication with families, guardians, and other stakeholders.
