Job Type: As-Needed/Flexible Hours
Location: Orange County Area
Responsibilities:
- Provide skilled nursing care to patients in their homes or hospice facilities
- Administer medications and treatments as prescribed by the physician
- Monitor and assess patient conditions, including vital signs and pain management
- Collaborate with interdisciplinary team members to develop and implement patient care plans
- Offer emotional support and education to patients and families
- Maintain accurate and up-to-date patient records and documentation
Requirements:
- Current LVN license in California
- Strong communication and interpersonal skills
- Ability to work independently and as part of a collaborative team
- Compassionate and empathetic approach to patient care
- Reliable transportation and willingness to travel to patient homes/ facilities
