Schedule: Day/Evening/Night - must be able to work all shifts. Must be able to orient for 24-36 hours per week until orientation is complete.

SUMMARY: The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans. The Licensed Practical Nurse is a key member of the care delivery team. The Licensed Practical Nurse communicates the needs of the patient and the team; collaborates with the multidisciplinary team, takes direction from the RN and delegates appropriately to the other members of the team.

The Acute Medical Unit (5800) specializes in the care of patients with a wide array of medical diagnoses and multi-system problems. LPNs working on 5800 will have the opportunity to master their skills in caring for patients with the following acute illnesses & conditions: cardiac requiring telemetry, diabetes, kidney disease, dialysis, pulmonary issues, and patients with multiple drips, drains & catheters. LPNs on 5800 will also possess strong psychosocial skills to support patients with substance abuse and/or mental illness, in addition to their medical needs.

RESPONSIBILITIES:

· Patient Care & Service - Promote and restore patients' health by completing the nursing process; collaborate with physicians and multidisciplinary team members; perform various treatment procedures; provide physical, educational and emotional support to patients, friends and families; supervise assigned team members.

· Medication Administration & Reporting - Administer medication and give/receive relevant reports, both written and verbal from/to appropriate staff as per departmental protocols.

· Documentation - Ensure concise, pertinent and complete documentation using computerized medical record process.

· Preparation - Ensure clinical areas are properly supplied and exam rooms are available and stocked.

REQUIRED QUALIFICATIONS:

· Graduate from an accredited School of Practical Nursing or equivalent required.

· Current licensure in State of New York as a Licensed Practical Nurse.

PREFERRED QUALIFICATIONS:

· Strong communication, customer service, and interpersonal skills highly desired

EDUCATION:

LICENSES / CERTIFICATIONS: 

BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - NYS Office of Professions

PHYSICAL REQUIREMENTS:

Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements.

PAY RANGE:

$27.54 - $33.66Rochester14621

The posted pay range is a good faith representation of potential base pay for this position. It may be modified in the future and eligible for additional pay components. Individual annual salaries/hourly rates could vary based on position details and FTE. Pay is determined by considering factors including, but not limited to, experience, relevant qualifications, specialty, internal equity, locations, and contracts.
