Experienced Home Health RN Leader Needed | Minimum 3 Years of Home Health Experience Required

Human Touch Home Health is seeking an experienced Registered Nurse (RN) Clinical Manager to lead and support our skilled home health clinical operations in Washington, DC.

This is not an entry-level management position.

We are looking for an RN who knows Home Health, understands the clinical and regulatory demands of the industry, and can confidently lead a team while maintaining the highest standards of patient care.

Candidates MUST have a minimum of 3 years of Home Health experience.

What You'll Do

As the RN Clinical Manager, you will provide clinical leadership and oversight to ensure patients receive safe, compliant, high-quality home health care.

Responsibilities include:

• Provide clinical oversight for skilled home health services

• Supervise and support RNs, LPNs, PTs, OTs, SLPs, MSWs, and other clinical team members

• Review Start of Care, Recertification, Resumption of Care, Transfer, and Discharge documentation

• Review OASIS assessments for accuracy, completeness, and consistency

• Ensure plans of care accurately reflect patient needs and physician orders

• Monitor clinical documentation for quality, timeliness, and regulatory compliance

• Ensure physician orders are obtained, documented, and followed appropriately

• Participate in case conferences and interdisciplinary care coordination

• Monitor patient outcomes, hospitalizations, and changes in condition

• Address clinical concerns and provide guidance to field clinicians

• Support orientation, training, coaching, and ongoing development of clinical staff

• Monitor clinician performance and documentation standards

• Assist with quality assurance and performance improvement initiatives

• Support survey readiness and corrective action efforts

• Ensure compliance with agency policies and applicable federal and DC requirements

• Collaborate with Intake, Scheduling, Billing, Recruitment, and Operations

• Help maintain appropriate clinical staffing and continuity of patient care

• Promote accountability while creating a supportive environment for the clinical team

What We're Looking For

• Active Washington, DC Registered Nurse License Required

• Minimum 3 Years of Home Health Experience REQUIRED

• Previous clinical leadership, supervisory, or management experience strongly preferred

• Strong knowledge of OASIS and skilled home health operations

• Understanding of Medicare home health requirements and clinical documentation standards

• Experience reviewing plans of care and clinical documentation

• Strong understanding of quality assurance and regulatory compliance

• Excellent clinical judgment and problem-solving skills

• Strong leadership, coaching, and communication abilities

• Ability to hold staff accountable while providing effective clinical support

• Excellent organizational and time-management skills

• Comfortable working in a fast-paced home health environment

• WellSky experience strongly preferred
