The Hospice RN Case Manager will have the opportunity to connect with our patients and positively impact their lives every day. They will promote patients' physical, personal, and emotional well-being, working with the Dover Health teams toward our ultimate goal which is to help each patient return to their maximum level of self-care and independence, when possible.


Essential Duties:

  • Provide and direct the provisions of nursing care, as prescribed by the physician, including assessment, diagnosis, planning, implementation, and evaluation within the guidelines of the standards of nursing care, and in compliance with the applicable laws, regulations, and agency policies.
  • Performs the initial and/or comprehensive nursing evaluation visit, identify the primary focus of care, develops an individualized plan of care in collaboration with other members of the interprofessional or interdisciplinary team, patient, family, or caregiver.
  • Use critical nursing skills to regularly assess and reassess the patient's ongoing needs, report changes in patients' conditions, such as change in condition, adverse reactions to medication or treatment, and provide necessary nursing intervention within the scope of nursing standards of care.
  • Educate patients and their families or caregivers about managing patient’s illness or injury, including home care needs and medication administration.
  • Participate in activities that promote patient safety, quality, and regulatory compliance.
  • Participate in professional development and lifelong learning activities.
  • Supervise the LPNs and Hospice CNAs.
  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is occasionally required to stand/move, communicate, and identify written information. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl and lift and/or move up to 50 pounds independently; occasionally lift and/or move up to 200 pounds with assistance.
  • Other duties as assigned.

Compensation & Benefits:

The pay range for this position is $87,000-$100,000 annually, depending on experience and qualifications. In addition, we offer a comprehensive benefits package, including:

Qualifications, Education and/or Experience:  

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skills, and abilities required.  

  • Diploma, Associate or Bachelor’s degree in nursing
  • Current RN license, specific to the state you are assigned to work.
  • Hospice or home care experience strongly preferred.
  • Willing to obtain hospice and palliative care certification within 1 year.
  • Current CPR certification.
  • Current and valid driver’s license, reliable transportation, and current liability auto insurance.
  • Ability to work well with others and take direction from management, as well as take initiative – willing to go above and beyond to ensure our patients needs are met and they are comfortable in their living environment.
  • Self-motivated to accomplish identified goals with a strong sense of accountability for results.
  • Be able to demonstrate proficiency in Internet browsers (e.g., Explorer, Chrome, and Fire Fox), Microsoft Office programs including Outlook, Excel, Word and Publisher, electronic health records (EHR) systems, and any HR related systems if applicable.

  • Frequent travel within assigned service area is required.
  • While performing the duties of this job, the employee is?occasionally required to stand for extended periods of time, move, communicate, and identify written information.?The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl and lift and/or move up to fifty (50) pounds independently, and occasionally lift and/or move up to two hundred (200) pounds with assistance.
  • Work for this position is performed in patient homes, care settings/facilities, and office settings. This position will have to navigate through a variety of different settings to provide care. 
  • There is exposure to varied home environments and potential infectious conditions. 
  • Must comply with all Dover Health and safety requirements.
  • This role involves actively participating in all staff activities aimed at fostering teamwork, unity, and morale. The individual will contribute as a collaborative team player, working alongside colleagues to create a supportive and cohesive work environment. 
  • Individuals in this position are required to stay current on all training and ongoing education initiatives. They are expected to actively pursue self-improvement and embrace opportunities for continuous learning to enhance their skills and knowledge in providing exceptional service.
