Responsibilities
Registered Nurse (RN)
As a Registered Nurse, you will provide skilled nursing care in a collaborative outpatient environment while helping patients achieve optimal health outcomes.
Key Responsibilities
- Deliver skilled nursing services under the direction of physicians and advanced practice providers
- Assess patient health needs and analyze clinical data
- Develop, implement, and evaluate individualized plans of care
- Monitor patient progress and modify care plans as appropriate
- Collaborate with providers and interdisciplinary healthcare teams
- Educate patients and families regarding health conditions, treatments, and preventive care
- Maintain a safe, therapeutic, and patient-focused environment
Registered Nurse Care Coordinator (RNCC)
As a Registered Nurse Care Coordinator, you will play a critical role in supporting patients across the continuum of care, with a focus on coordination, communication, and improving health outcomes.
Key Responsibilities
- Partner with providers and clinic teams to deliver evidence-based, patient-centered care
- Support patients in managing care plans and achieving health goals
- Coordinate care for high-risk and medically complex patients
- Assist patients with chronic conditions, disabilities, and disease-specific care management needs
- Facilitate smooth care transitions, including hospital discharge follow-up and transitions between care settings
- Serve as a liaison among patients, families, healthcare providers, and community resources
- Promote comprehensive, whole-person care through proactive communication and care coordination
Qualifications
Minimum Qualifications
Required for Both Positions
- Associate Degree in Nursing from a CCNE or ACEN accredited nursing program
- Current Ohio Registered Nurse license
- Current CPR/BLS certification
- Experience in clinic, physician office, urgent care, or related healthcare settings
- Knowledge of triage procedures and patient care standards
- Strong communication and patient education skills
- Ability to maintain quality, safety, and infection control standards
- Excellent interpersonal, organizational, and critical-thinking skills
- Ability to maintain confidentiality and handle sensitive situations with professionalism and diplomacy
