What You'll Do
Support Resident Well-Being
- Coordinate services and resources to address the psychosocial needs of Residents in Residential Living.
- Partner with the Manager of Resident Services to address Resident concerns and develop appropriate support plans.
- Participate in regular meetings with leadership to discuss Resident interactions and emerging needs.
Guide Residents Through Life Transitions
Assist Residents and families in navigating significant life changes, including:
- Chronic illness or declining health
- Changes in mobility or activities of daily living
- Loss of driving privileges and transportation planning
- Home care arrangements
- Temporary or permanent moves to Supportive Living
- Hospitalizations
- Loss of a spouse or loved one
Provide compassionate guidance and connect Residents with internal and external support resources.
Collaborate Across the Continuum of Care
- Work alongside Residents, spouses, families, physicians, nursing teams, administrators, chaplains, and Continuing Care Assessment Team Members to ensure smooth transitions and personalized care planning.
- Consult with Willow Valley admission and healthcare teams regarding discharge planning following hospitalizations.
- Support hospitalized Residents and their spouses by helping address psychosocial and emotional needs.
Foster Positive Community Relationships
- Welcome and assist new Residents as they become familiar with the community.
- Help resolve Resident-to-Resident conflicts through mediation, communication, and problem-solving techniques.
- Promote a supportive and respectful community environment.
Coordinate Resident Transitions
- Maintain knowledge of Resident Agreements, Resident Handbooks, and contractual requirements.
- Lead communications with Residents and responsible parties during transitions from Residential Living to Supportive Living.
- Coordinate appropriate communication and support when a residence is vacated due to relocation or the passing of a Resident.
What We're Looking For
Required Qualifications
- Bachelor's or Master's Degree in Social Work from an accredited college or university.
- Strong interpersonal, communication, and relationship-building skills.
- Ability to navigate sensitive situations with professionalism, empathy, and discretion.
- Excellent organizational and problem-solving abilities.
- Commitment to person-centered service and collaborative teamwork.
