The Impact You'll Make
As a Licensed Practical Nurse, you’ll be the heartbeat of our patient care team. You’ll provide hands-on nursing support, build trusting relationships with patients and families, and ensure that every individual receives the highest standard of care. Your role will directly influence patient comfort, recovery, and overall well-being.
What You'll Do
- Provide direct patient care, including administering medications, wound care, and monitoring vital signs.
- Assist in developing and implementing individualized care plans in collaboration with RNs and physicians.
- Support patients with daily living activities, ensuring dignity and respect at every step.
- Document patient information accurately in electronic health records (Hospice Tools) systems.
- Educate patients and families on treatment plans, medications, and healthy lifestyle choices.
- Respond promptly to patient needs and changes in condition, escalating concerns when necessary.
- Maintain a clean, safe, and organized care environment in compliance with health and safety regulations.
What You'll Bring
- Current LPN license in Minnesota and in good standing.
- Proven experience in a clinical, hospital, or long-term care setting.
- Strong communication and interpersonal skills, with a compassionate bedside manner.
- Proficiency in Hospice Tools systems and basic medical technology.
- Ability to work collaboratively in a fast-paced, team-oriented environment.
- A commitment to patient-centered care and continuous learning.
