SCOPE AND PURPOSE OF POSITION:
This position works within the Health Home Care Management Program to support program leadership and field-based care managers. The role provides care managers with support for non-billable case activities and performs multiple administrative and operational tasks to help achieve program objectives. The position also assists the Director and/or Assistant Director with program reporting, data collection, tracking, and other activities necessary to monitor program performance and outcomes.

Responsibilities
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Completing non-billable case management support activities, including sending discharge paperwork, coordinating and sending meeting invitations, and retrieving appropriate documentation from electronic databases.
- Providing administrative support to field-based care managers, including reviewing, scanning, and uploading documents into the appropriate electronic health records and databases.
- Supporting program leadership with tracking staff training attendance, maintaining training records, and ensuring required documentation is completed and organized.
- Supporting the Director with compliance activities, including tracking required documentation, monitoring completion, and assisting with follow-up as needed.
- Performing additional administrative and program support tasks as needed to assist leadership and care management staff in meeting program requirements and objectives.
- Assisting with onboarding activities for new staff and supporting onboarding needs related to new program locations.
- Assisting with program data collection, analysis, tracking, and reporting to support program performance and objectives

Qualifications
- Bachelor’s Degree and 3-5 years in social services program. Case management/health home experience preferred.
- Ability to work in multiple electronic health records efficiently.
