Overview

The Care Palliative Coordinator (CPC) is responsible for facilitating a seamless transition for patients discharging from a facility setting to palliative care. The CPC meets patients, reviews goals for care and ensures continuity of care and the ability to meet the needs of the patient as they transition.

Responsibilities

  • Execute a weekly, monthly, and quarterly strategy to increase market share through key account development including prospecting diversification and call frequency routing. Plan activity to maximize territory coverage of both existing and prospective accounts.
  • Achieve care transition goals and expectations as established at hire or at review of quarterly agency budget goals.
  • Assist with preparation for accepting care of the patient post discharge.
  • Coordinate other services for the patient with ancillary service providers (DME/Infusion).
  • Coordinate the collection, organizing, and transfer of pertinent information for the staff.
  • Complete goals of care discussion with patient and caregivers while under patient care or in the clinic or community setting.
  • Explain services and procedures to the patient and his/her family members.
  • Involve the family/caregivers in the educational process and assess post-discharge education/coaching needs.
  • Follow patients at the hospital when readmitted.
  • Participate in weekly palliative IDG meeting and if appropriate, give an update regarding status. Assess customer needs and progress toward growth strategies.
  • Schedule follow-ups call 48 hours post admit.
  • Serve as liaison between practice, facility care setting and referral source.
  • Provide education information for hospital staff and physicians regarding palliative care.
  • Visit and communicate with the patient in the hospital to obtain necessary information to facilitate care transition.

Qualifications

  • One-year medical care setting experience.
  • Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) or Social Worker (SW) licensure in state of practice.
  • Experienced professional in medical services.
  • Driver's license, valid vehicle insurance and access to a dependable vehicle or public transportation
  • Excellent organizational skills
  • Excellent verbal and written communication skills
  • Proficient computer skills.
  • Travel 25-75%
