· Establish and maintain agreements, referral processes, and communication protocols to ensure seamless service integration for families.

Family Services Coordination & Support

· Provide consultation and guidance on complex family cases, including crisis situations, safety planning needs, and multi-system involvement.

· Support the Family Support Specialist in developing individualized care plans, ensuring appropriate service matching and family-centered approaches.

· Assist with escalated service coordination tasks, including collaboration with providers on behavioral health, medical, educational, and social service referrals.

· Ensure families receive trauma-informed, culturally responsive, developmentally appropriate care throughout the continuum of services.

· Oversee and support aftercare planning processes to reduce recidivism and promote long-term stability for families.

Training, Compliance & Quality Assurance

· Conduct regular audits of case records, care plans, progress notes, and referral documentation to ensure compliance with agency policies, HIPAA, and applicable laws.

· Track and respond to critical incidents, complaints, or concerns, implementing corrective action when necessary.

· Monitor program trends and emerging best practices to enhance service quality and efficiency.

 Administrative & Other Duties

· Support the Director of Children and Family Services with program planning, grant reporting, and evaluation activities.
