Overview
Are you passionate about helping patients navigate their healthcare journey? We are seeking a compassionate, detail-oriented Therapy Referral Coordinator to join our team. In this role, you will play a vital part in coordinating therapy and specialty care referrals, ensuring patients receive the services they need in a timely and efficient manner.
Responsibilities:
- Assist physician and nurse in preparing therapy referrals in a timely manner.
- Work closely with therapy providers to provide necessary referrals.
- Verify Medicaid eligibility/PCP assignment for any non-same day request and review complete eCW records to verify appropriateness of referral
- Document all communication and maintain accurate documentation in Ecw.
- Perform any other related duties as required or assigned.
Qualifications:
- High school diploma or GED
- 12 to 18 months related experience and/or training
