Pay Rate: $36.00–$40.00 per hour

Pleasant Hill Post Acute is seeking a compassionate, dependable, and dedicated Licensed Vocational Nurse (LVN) to join our clinical team on a Part-Time / On-Call basis.
The ideal candidate is committed to providing safe, high-quality, resident-centered care while working collaboratively with residents, families, physicians, and the interdisciplinary healthcare team.
Responsibilities
- Provide nursing care and services to residents in accordance with established care plans.
- Administer medications and treatments as ordered.
- Monitor residents' conditions and promptly report changes to the appropriate healthcare professionals.
- Complete accurate and timely nursing documentation.
- Assist with assessments and contribute to individualized care planning.
- Communicate effectively with residents, families, and members of the healthcare team.
- Follow infection control, safety, and facility policies and procedures.
- Promote residents' comfort, dignity, and overall well-being.
Qualifications
- Current and valid California LVN license.
- Previous skilled nursing or post-acute experience preferred.
- Strong communication and organizational skills.
- Dependable, compassionate, and team-oriented.
- Ability to work effectively in a fast-paced healthcare environment.
