Compensation

We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our growing home care team on a per diem basis. This position offers independence, flexibility, and meaningful one-on-one care for patients who prefer to remain in the comfort of their homes.

Service areas currently extend throughout the Lakes Region and Central New Hampshire, with opportunities ranging from Laconia, Belmont, and Gilford south through Tilton, Franklin, and Concord.

Position Overview

The LPN will provide skilled nursing visits focused on promoting recovery, maintaining stability, and enhancing quality of life. This is an ideal role for nurses who thrive in autonomous settings and want flexible scheduling with competitive per-visit compensation.

Key Responsibilities

• Conduct in-home patient assessments, medication management, and wound care

• Monitor and document changes in patient condition and report to supervising RNs or physicians

• Educate patients and caregivers on treatment plans, medications, and home safety practices

• Maintain accurate, timely documentation using electronic charting

• Collaborate with multidisciplinary care teams to ensure coordinated and effective patient care

Requirements

• Current and active LPN license in the State of New Hampshire required

• Valid driver's license and reliable transportation

Visits may be assigned throughout the Belmont, Laconia, Gilford, Tilton, Franklin, and Concord areas based on your availability and preferred travel radius.
