OSU Comprehensive Cancer Center and The James Cancer Hospital (OSUCCC-James) are responsible for implementing the Outreach Strategic Plan for Cancer. Developing and implementing a community oncology network is critical for the sustained future success of the OSUCCC-James and is a key strategy in the Cancer Program Strategic Plan. Central to this plan are the efforts required to design and build an oncology network that contractually secures affiliate relationships and increases the quality of cancer care in the communities being served. The James Cancer Hospital at The Ohio State University Wexner Medical Center is committed to enhancing the quality of care provided to patients. The Care Coordinator will work within The James and collaborate with affiliate members of the James Cancer Network (JCN) program for patients with a cancer diagnosis that are being seen mutually between the James Cancer Network site and The James Cancer Hospital. The Care Coordinator will be responsible for facilitating communication between patients and providers (external and internal) through the various transitions of their disease. This individual may also perform a brief initial screening of patients for clinical trial eligibility.
Position Summary
Under the direction of the Care Coordination Manager, the Care Coordinator will be to serve as a point of contact for cancer patients who are referred or come as a self-referral from James Cancer Network affiliate sites for consultation or care at The James. The Care Coordinator will collaborate with multidisciplinary providers and professional staff within and across The James and serve as a liaison to facilitate on-going communication between patients and both internal and external providers. The Care Coordinator will provide support and education to patients and their caregivers related to issues that may occur when facing a cancer diagnosis.
Minimum Qualifications
Current Ohio nursing license required
Bachelor’s Degree in Nursing preferred
Oncology Nursing Certification (OCN) preferred
Minimum of 4-6 years’ oncology experience preferred
Basic PC and keyboard skills required; previous experience with MS office products (Word, Excel, and PowerPoint)
Communicate effectively in English (verbal/written)
