What You'll Do!!
As a Home Health LPN, you'll provide skilled nursing care under the direction of the RN Case Manager and attending physician while helping patients safely recover at home.
Your responsibilities will include:
Providing skilled nursing visits in patients' homes
Monitoring patient conditions and communicating changes to the care team
Administering medications and treatments
Educating patients and families on disease management and preventative care
Helping reduce hospital readmissions through quality patient education
Collaborating with physicians, case managers, and interdisciplinary team members
Completing timely documentation in accordance with Medicare and agency guidelines
What We're Looking For
Current Licensed Practical Nurse (LPN) license
Minimum of one year of acute care LPN experience (two years preferred)
Home health experience preferred
Valid driver's license and active auto insurance
Strong assessment, communication, and documentation skills
Compassionate, dependable, and patient-focused
