Make a Difference Through Home-Based Rehabilitation

Join our Home Health team as a Physical Therapist Assistant (PTA) and help patients regain strength, mobility, and independence in the comfort of their homes. Working under the supervision of a Physical Therapist, you will provide skilled treatment services, educate patients and caregivers, and collaborate with an interdisciplinary team to achieve the best possible outcomes.

Position Summary

The Physical Therapist Assistant provides therapeutic interventions and rehabilitation services in the home setting according to the established plan of care. This role focuses on helping patients improve functional mobility, safety, and quality of life while ensuring exceptional patient-centered care and clinical excellence.

Key Responsibilities

Patient Care & Treatment

  • Deliver skilled physical therapy treatments as outlined in the plan of care established by the supervising Physical Therapist.
  • Monitor patient progress and report changes in condition, functional status, or treatment response.
  • Recognize the need for reassessment and communicate clinical concerns to the supervising therapist.
  • Utilize evidence-based interventions to help patients achieve established goals and improve independence.
  • Promote patient safety and reduce risk of injury during treatment sessions.

Patient & Family Education

  • Educate patients and caregivers on exercises, mobility techniques, safety precautions, fall prevention, and disease management.
  • Assess learning needs and adapt teaching methods to support health literacy and individual learning styles.
  • Empower patients and families to safely manage care and maximize functional outcomes at home.

Care Coordination & Team Collaboration

  • Collaborate with Physical Therapists, nurses, physicians, and other healthcare professionals to ensure coordinated care.
  • Participate in interdisciplinary case conferences and care planning activities as appropriate.
  • Support discharge planning and transition of care efforts.
  • Maintain effective communication using established clinical communication standards and best practices.

Documentation & Quality

  • Complete timely, accurate, and compliant clinical documentation.
  • Support quality improvement initiatives and organizational performance goals.
  • Maintain compliance with regulatory, accreditation, and reimbursement requirements.
  • Utilize resources effectively while providing high-quality, cost-efficient care.

Professional Development

  • Participate in staff meetings, educational programs, and ongoing training opportunities.
  • Maintain current knowledge of therapy practices, clinical guidelines, and industry standards.
  • Demonstrate professionalism, teamwork, and a commitment to continuous learning.

Qualifications

Required Licensure & Certifications

  • Current Illinois Physical Therapist Assistant (PTA) license or eligibility for Illinois licensure.
  • Valid Illinois driver's license and reliable transportation.
  • BLS Certification through the American Heart Association (or obtained within six months of hire if required by department leadership).

Required Education

  • Graduate of an accredited Physical Therapist Assistant program.

Preferred Experience

  • Two years of clinical experience preferred, or strong clinical affiliation experience demonstrating readiness for independent practice in the home health setting.

Knowledge & Skills

  • Strong communication and interpersonal skills.
  • Excellent time management, organization, and documentation abilities.
  • Ability to work independently and manage a patient caseload across the assigned territory.
  • Basic computer proficiency, including electronic documentation systems, Microsoft Office, and email.
  • Knowledge of home therapy equipment, assistive devices, and safe patient handling techniques.

Physical Requirements

  • Frequent travel throughout the assigned service area.
  • Ability to lift and carry up to 35 pounds independently and assist with patient transfers using appropriate equipment and support.
  • Ability to bend, reach, stand, walk, climb stairs, and provide treatment in various home environments.
  • Ability to work occasional weekends and holidays based on patient care needs and team rotation.
