Medical Social Worker (MSW) – Home Health
Now Hiring Licensed MSW
Southwest General Home Health Care and Hospice
Schedule: Per diem 1 day a week
5-6 patients a week
Territory: Macedonia, Twinsburg, Parma, Cleveland and surrounding areas

s a Medical Social Worker (MSW), you will:
- Complete comprehensive patient assessments.
- Participate in developing individualized Plans of Care.
- Identify barriers to care and recommend appropriate interventions.
- Connect patients and families with valuable community resources.
- Assist with financial assistance applications and long-term planning.
- Educate and support patients and caregivers throughout their care journey.
- Provide counseling related to adjustment, coping, loss, and grief.
- Collaborate with physicians, nurses, therapists, and the interdisciplinary care team.
- Complete accurate clinical documentation in accordance with agency, Medicare, and federal guidelines.
- Observe, document, and communicate changes in patient condition.
- Attend required training and agency meetings.

Qualifications
- Current Medical Social Worker (MSW) license in the state of employment (limited licenses not accepted)
- Master's Degree in Social Work (MSW) required
- Minimum of one year of MSW experience in an acute care setting
- Home health experience preferred
- Valid driver's license and current auto insurance
- Ability to meet the physical demands of the position
