Description

Sheba Consulting is helping establish a primary care family medicine practice with a concierge model for a physician assistant with 20 years of experience. We are seeking a board-certified Family Medicine MD or DO to provide remote supervision and clinical collaboration for one PA and serve as the named physician owner of the practice's dedicated legal entity. Sheba and the management services organization (MSO) will handle business setup and administration.

The practice will focus on diagnostics, complex chronic disease management, preventive care, women's health, GLP-1 management, and lifestyle medicine. Patient care will be delivered primarily through telehealth, with occasional home visits by the PA. The physician's role is remote supervision, consultation, and PC ownership; no separate patient panel is expected.

Responsibilities
  • Provide clinical supervision and consultation for one experienced PA serving patients in New Jersey and New York under a written supervision/collaboration agreement.
  • Participate in periodic chart review, clinical case discussion, and an agreed process for questions and escalation.
  • Serve as the named physician owner of a new professional corporation (PC) dedicated to this practice, supported by the MSO. An existing PC shared with other MSOs will not meet the planned structure.
  • Work with healthcare counsel and Sheba on the agreements defining the physician's clinical and legal ownership responsibilities.
  • Maintain appropriate licensure and malpractice coverage for the role, with reliable availability under an agreed coverage protocol.
Required qualifications
  • MD or DO with current board certification in Family Medicine.
  • Active, unrestricted medical licenses in both New Jersey and New York, in good standing, with no disciplinary actions, probation, or restrictions.
  • Your own malpractice coverage appropriate to the proposed supervision and ownership responsibilities.
  • Willingness to undertake both PA supervision and ownership of the dedicated PC.
  • An evidence-based approach to family medicine, sound clinical judgment, and clear, responsive communication.

Preferred experience

Experience supervising PAs and managing complex chronic conditions, women's health, and lifestyle medicine. A Florida license, or willingness to obtain one, is helpful for possible future expansion; Florida is outside the current engagement.

Engagement

Compensation is $500-$1k per month, depending on skills, experience, and suitability, for combined supervision and PC ownership. The fee is independent of revenue or patient volume. Formation and filing costs will be paid by the practice. Time commitment, coverage expectations, start date, and final terms will be agreed before engagement.

Sheba will coordinate entity formation and administrative management. Healthcare counsel will prepare the practice documents; the physician will retain responsibility for the clinical and ownership duties in the final agreements.
