Job Description

Participates as a member of an interprofessional team in completing selected services and functions related to patient care coordination, discharge placements, community referrals, and data collection under the supervision of a nurse case manager or a social worker. Maintains necessary documentation and communication with internal and external customers. Proactively supports efforts that ensure delivery of safe patient care and services and promote a safe environment at Children's Healthcare of Atlanta.

Experience

  • 1 to 3 years in a health care setting

Preferred Qualifications

  • Associate degree in a healthcare-related field
  • Strong communication and interpersonal skills – written and verbal
  • Critical thinking and problem-solving skills
  • Knowledge of and ability to utilize internal and external resources
  • Flexibility in prioritization and effective organizational skills
  • Ability to multitask and function in a stressful, fast-paced environment
  • Experience with charting in Epic

Education

  • High school diploma or equivalent

Certification Summary

  • No professional certifications required

Knowledge, Skills, and Abilities

  • Knowledge of medical terminology
  • Knowledge of Microsoft Office (Word, Excel, and PowerPoint)

Job Responsibilities

  • Works in collaboration with the Case Management and Social Work teams in activities related to discharge placement, care coordination, and community referrals.
  • Under the supervision of a case manager, social worker, or other licensed provider, obtains, sends, and/or faxes patient information to external agencies or providers following hospital policy related to confidentiality.
  • Accurately and promptly documents interventions appropriately. Develops and maintains proficiency in the use of computer software for charting, data entry, and other duties.
  • Communicates professionally and establishes effective working relationship with internal and external customers. Performs multiple, concurrent tasks in frequently stressful and fast-paced environments while maintaining strong internal and external customer service standards.
  • Completes follow up calls to clients and their care providers at the request of the case manager, social worker, or other licensed provider. Can elicit information during follow up calls and understands when to have a case manager, social worker, or other licensed provider intervene.
  • Coordinates appointments with physicians, testing, and other services when appropriate. Schedules follow-up appointments as justified by physician order.
  • Provides verbal and written confirmation of appointments to patients/families. Documents the patients'/families' agreement and understanding of schedule. Faxes appointment confirmations to the appropriate community provider.
  • Assists within a discharge timeframe and with appropriate discharge placement.
  • Issues and documents Medicare or other communication provided to patient/families (e.g., IML for Medicare beneficiaries) as needed.
  • Participates in maintaining and monitoring the work environment to ensure a continual state of CMS and Joint Commission readiness.
  • Escalates problems related to completing assigned duties in a timely and effective manner.
