Job Summary and Responsibilities

CHI St. Joseph’s Health Community Health is seeking a passionate and organized individual to join our team as a Grant Coordinator. This position is instrumental in driving community-driven solutions to improve health outcomes across the North Country (Clearwater, Hubbard, Lake of the Woods) and Beltrami County Community Health Boards.As a Grant Coordinator at CHI St. Joseph's Health Community Health, you will be a vital bridge between public health initiatives and our local communities. You will be responsible for the coordination of the Statewide Health Improvement Partnership (SHIP) grant, working to implement sustainable policy, system, and environmental changes that prevent chronic diseases such as cancer, heart disease, and diabetes.Every day, you will engage with diverse settings—including schools, workplaces, childcare centers, and healthcare sites—to foster environments that support active living, healthy eating, and commercial tobacco-free living. You will serve as a facilitator for Local Community Leadership Teams, leveraging your ability to build and maintain strong, collaborative partnerships to turn grant objectives into impactful community realities.To thrive in this role, you will need a blend of professional relationship-building skills, grant management expertise, and a deep commitment to public health. You are a strategic thinker who can navigate complex community systems, effectively communicate health-improvement goals to stakeholders, and manage grant deliverables with precision and transparency.Responsibilities:

• Coordinate the implementation of SHIP grant activities across the North Country and Beltrami County CHB service areas, focusing on healthy eating, active living, and tobacco-free initiatives.
• Cultivate and maintain strong, long-term partnerships with community leaders, school districts, employers, and healthcare organizations to drive local health-improvement strategies.
• Facilitate meetings with Local Community Leadership Teams to solicit input, provide project updates, and ensure all grant interventions are community-driven and locally relevant.
• Monitor and track grant-funded progress, ensuring all activities align with state requirements and organizational goals for reducing chronic disease.
• Provide technical assistance to community sites to implement environmental changes, such as increasing access to fresh foods, improving physical activity infrastructure, and establishing smoke-free policies.
• Bridge the gap between clinical settings and community resources by strengthening referral pathways for patients to access evidence-based programs in their area

Job Requirements

• Bachelor of Science in Public Health, Community Health, Health Education, or a related field OR Bachelor of Science to be obtained within one (1) year of employment. 
• Valid Minnesota Driver's License. 
• Exceptional communication and interpersonal skills, with the ability to lead and motivate diverse groups of stakeholders.
• Understanding of policy, systems, and environmental (PSE) change strategies and the social determinants of health.
• Ability to travel throughout the assigned counties (Clearwater, Hubbard, Lake of the Woods, and Beltrami) as needed.
• Grant Status: This position is fully grant-funded; continued employment is contingent upon the availability of grant funding.

 

Preferred:

• Experience in grant coordination, project management, or community organizing is highly preferred.
