What You’ll Do
As a Home Health LPN, you’ll be at the heart of patient care, bringing clinical expertise and compassion directly into the home. You will:
- Implement plans of care initiated by the registered nurse
- Provide accurate and timely documentation consistent with the plan of care
- Collect assessment data and perform observation to report to and collaborate with RN(s) and physician(s)
- Participate in coordination of home health services, appropriately reporting identified needs for other disciplines
- Participate in personal and professional growth and development
- Perform other duties as assigned by the registered nurse

What We’re Looking For
- Current LPN license in the State of Arizona
- Valid driver’s license and reliable, insured vehicle
- Current Fingerprint Clearance Card or the ability to apply
- Strong clinical skills and a passion for patient-centered care
- Ability to work independently and make a real difference
