Isaiah House is Kentucky’s largest nonprofit, Christ-centered addiction recovery organization. We provide evidence-based, holistic care to individuals battling substance use disorders. With a heart for transforming lives, we serve individuals and families across the Commonwealth through a full continuum of care, from residential treatment to outpatient services and recovery support.


 

Isaiah House is seeking a self-motivated and compassionate Client Care Navigator (CCN) to connect individuals, families, and community referral partners with our treatment and recovery services.

This position will be based in Franklin County and will serve Franklin, Shelby, Spencer, Bullitt, Hardin, and Henry Counties


Regular travel throughout the assigned region is required.


The ideal candidate is outgoing, organized, relationship-driven, and passionate about helping individuals affected by substance use disorder access the care they need.

 

Essential Job Functions

  • Develop and maintain strong professional relationships with Centers of Influence (COIs), community partners, and referral sources throughout the assigned region.
  • Educate hospitals, courts, correctional agencies, healthcare providers, community organizations, and other referral sources on Isaiah House programs, services, and treatment options.
  • Respond promptly and professionally to inquiries from individuals and families seeking treatment and provide guidance throughout the referral and admissions process.
  • Conduct ongoing outreach and follow-up with current and prospective referral partners to strengthen relationships and increase access to treatment services.
  • Plan and manage travel, outreach activities, and appointments throughout the assigned territory.
  • Maintain accurate and timely documentation of outreach activities, referrals, admissions, and related information using company systems.
  • Communicate new referrals, updates, and relevant information to the CCN Supervisor and appropriate team members in a timely manner.
  • Represent Isaiah House professionally at community events, meetings, presentations, networking opportunities, and outreach initiatives.
  • Collaborate with Admissions and other departments to coordinate referrals and support individuals in accessing appropriate treatment services.
  • Maintain confidentiality, professional boundaries, and compliance with organizational policies and applicable requirements.
  • Perform additional duties and responsibilities as assigned.


Requirements

  Qualifications

  • High school diploma or GED required; associate or bachelor's degree preferred.
  • Experience in substance use disorder treatment, behavioral health, healthcare, community outreach, admissions, case management, or relationship development preferred.
  • Strong communication, interpersonal, and relationship-building skills.
  • Self-motivated with strong organizational and time-management skills.
  • Ability to work independently and manage an assigned territory.
  • Valid driver's license, reliable transportation, and ability to travel regularly throughout the assigned region required.
  • Basic proficiency with Microsoft Office, Microsoft Teams, and electronic tracking systems preferred.
