Grand Peaks is looking to hire a full time Care Coordinator to join our Idaho Falls Medical team. This role is a good fit for someone who enjoys working with patients, building relationships, and helping people navigate the healthcare system. You’ll help coordinate referrals, follow-up care, and services while helping patients understand their care and overcome barriers along the way.
Full-Time | Monday–Friday | 8:30 AM–4:30 PM | No Weekends | Pay: DOE
Job Duties:
- Coordinate care for patients with chronic conditions and complex or high risk healthcare needs.
- Serve as a point of contact for patients and families, helping them navigate healthcare services and address questions or concerns.
- Coordinate referrals, appointments, medical records, and communication between primary care providers, specialists, hospitals, and community resources.
- Follow up with patients after emergency department visits and hospitalizations to support timely follow up care.
- Help patients understand their care plans, medications, referrals, and health goals.
- Identify barriers to care and connect patients with resources such as transportation, prescription assistance, interpreters, financial support, and community services.
- Communicate with patients by phone and in person to provide education, answer questions, and monitor progress.
- Maintain accurate documentation in the electronic medical record.
- Work closely with providers and the healthcare team to support coordinated, high quality care.
Requirements
- 2+ years of experience working with patients with chronic conditions, families, or other high-need populations preferred.
- Previous care coordination or care management experience in a medical setting is preferred.
- Strong communication, organization, problem-solving, and follow-through skills.
- Ability to manage multiple priorities and work independently.
- Knowledge of medical terminology and electronic health records.
- A compassionate, respectful, and patient-centered approach to care.
- Bilingual in English and Spanish preferred but not required
